Understanding the Context

Beau’s surgery team at UI Stead Family Children’s Hospital performed the first posterior tracheopexy in Iowa to permanently correct the weak trachea. Posterior tracheopexy is a valuable surgical technique for the treatment of tracheomalacia (TM) or the reduction in respiratory morbidity following TEF/EA repair in select neonates. Anterior tracheopexy using an adapted ringed PTFE prosthesis, in combination with a posterior tracheopexy, offers good short- and midterm results in young patients with severe tracheomalacia. The aim of this study is to evaluate the feasibility of robot-assisted posterior tracheopexy (RAPT), and to analyze the results of this minimally invasive approach in the treatment of TBM.

Introduction: Posterior tracheopexy (PT) directly addresses the posterior trachealis membrane intrusion in severe tracheomalacia. During PT, the esophagus is mobilized and membranous trachea is sutured to the prevertebral fascia. Posterior tracheopexy was performed by passing autologous pledgeted polypropylene sutures into butnot through theposterior tra-cheal membrane, and securing them to the anterior longitudinal spinal ligament under direct bronchoscopic guidance. This article outlines the progression from initial use of aortopexy, through secondary posterior tracheopexy associated with significant perioperative complications, to the establishment of PPT, highlighting lessons learned during this development.
